FA to launch investigation into tragic death of Sheffield United women's player Maddy Cusack after family provided new information - despite inquest clearing club of any wrongdoing

Cusack died at her home in Horsley, Derbyshire, on September 20, 2023, with police saying there were no suspicious circumstances surrounding her death. An independent third party found no evidence of wrongdoing at Sheffield United, despite the Cusack family submitting a complaint detailing a wide range of grievances relating to her past seven months at the club. Now, the FA are looking into the matter after her family submitted fresh information.

Cusack, who died at the age of 27, was the first player to reach 100 appearances for the Blades. Club manager Jonathan Morgan stepped away from his role last October while the investigation into her death took place but after United said no evidence of wrongdoing had been found, he returned to work earlier this month. Her family said Cusack's spirit had been "broken" in the months before her passing and that the midfielder would "still be with us" had Morgan not been appointed.

Cusack was an England under-19 international and also had stints with Aston Villa, Birmingham City, and Leicester City before moving to Sheffield United in 2019. She signed a one-year contract with the club last summer and a month later was named vice-captain, too.

Following her death, tributes were paid at football matches across the country, including at United's women's and men's games and England's fixture against Scotland. Now, the FA will begin their investigation into this matter.
