The Blues captain starts in his final game at Stamford Bridge against the bottom of the table team

John Terry has been handed the opportunity to say his farewells to Chelsea by starting the Premier League champion's final match of the season against Sunderland.

Conte set to buy house in Italy

He will make his 717th appearance for the club that he joined when he was 14-years-old. After the game, Chelsea will be presented with the Premier League trophy, which will be jointly lifted by the club captain and Gary Cahill.

Willian and Cesc Fabregas start ahead of Pedro and Nemanja Matic, as competition for places heats up in Chelsea's squad ahead of the FA Cup final against Arsenal next Saturday.

Chelsea line up in a 3-4-3 formation in what is virtually their strongest side, as they aim for a record 30th win in a Premier League season.

Terry wrote in his final programme notes ahead of the match: "These are my last captain's notes at Chelsea. Even writing that is difficult for me, but I can't tell you how happy I am to be going out at the Bridge with a title celebration."
