Leighton Baines will tell Everton he wants to leave if the club fail to qualify for the Champions League.
The left-back has been in excellent form so far this season as Everton challenge for a top-four finish in the Premier League and believes this summer could represent his last chance of a transfer to a top club.

Baines, 28, attracted interest from Manchester United last summer but Sir Alex Ferguson was deterred by Everton’s £15 million asking price as he looked for a long-term replacement for Patrice Evra.
The England international is under contract until 2015 and Everton are likely to demand £20m in the summer for a player recently described by his manager David Moyes said was ‘as good a left-back as there is'.
Baines’ international ambitions have been hampered by being behind Ashley Cole in the pecking order and the former Wigan man believes he needs to play for a club in the Champions League if he is to push for a starting role at the 2014 World Cup in Brazil.
Everton are currently sixth in the Premier League table after their defeat to Manchester United on Sunday left them six points behind fourth-placed Tottenham.
Baines has made more than 200 appearances for Everton since moving to Goodison Park from Wigan in a £6m deal in 2007 and has made 14 appearances for England, scoring one international goal.
