After taking charge of Chelsea's penalty shoot-out victory over Bayern Munich in the Champions League showpiece in May, the game in Kiev be his second major final in the space of six weeks.
Proenca, who became a Fifa-listed referee in 2003, has already taken charge of three matches at Euro 2012 - his first major international competition.
He whistled Spain's 4-0 victory against Republic of Ireland and Sweden's 2-0 win over France, before officiating the quarter-final contest between England and Italy, which the Azzurri won on penalties.
The 41-year-old will be assisted by Bertino Miranda and Ricardo Santos, with Turkish referee Cuneyt Cakir acting as the fourth official.
