A trophy presentation and further farewells from Terry will follow at the Bridge, but that's all from this Premier League season. Brilliant, brilliant Chelsea have ended a glorious campaign on a high.
Terry bowed out with a fitting tribute, too, while Courtois takes the Golden Glove courtesy of Hull City scoring against Tottenham elsewhere.
Chelsea have win number 30 - and in some style. Goals from Willian, Hazard, Pedro and Batshuayi, twice, made light work of Sunderland, who had scored through Manquillo.

Manquillo scored early for Sunderland, but Willian quickly levelled as Chelsea dominated long stretches of the half. Perhaps the most notable moment, though, came when Terry bade farewell to the Bridge with a 26th-minute substitution.

36' While the rest of the Chelsea cast enjoy themselves, Diego Costa has looked a little sluggish. He forces a corner, but only after miscontrolling as he looked to shoot. Pickford takes the set-piece easily.
34' Willian crosses, Diego Costa stretches, but Jones shields the ball and Pickford gathers at the second attempt.
33' Again Sunderland struggle to clear, but Hazard cannot force his way through and Pickford and his back-line can breathe again.

Pickford sportingly kicks the ball out in minute 26 - the same as the Chelsea captain's squad number - and Terry is quickly surrounded by team-mates offering their best wishes. He is given his own guard of honour and, clearly emotional, passes the armband to replacement Gary Cahill.
25' And again Sunderland cross from the right. This clearance is poor, allowing Bryan Oviedo to keep the attack alive. The ball breaks for Cattermole, whose effort is even worse this time and drags wide.
24' An energetic Rodwell burst gives Sunderland further respite, but, as he reaches the right-hand corner flag, his cross is nodded away.
23' Januzaj holds the ball up on the left for Sunderland, finally allowing them to enjoy some possession. Lee Cattermole takes the pass, though, and lashes over from distance.
21' Still Sunderland struggle to clear. The ball comes straight back and Hazard tees up David Luiz. Yet another deflection, yet another corner. David Luiz meets this cross, too, but nods well wide.
20' CHANCE! Fabregas' corners are twice cleared at the near post, before Alonso gets in to cross. Diego Costa cannot get a shot off, but Moses can and sees his effort deflected over the top.
19' This is lovely stuff from Chelsea. Hazard and Fabregas trade passes on the left and suddenly Terry appears on the overlap. He wins a corner.
18' Larsson's delivery is deep, towards the far post and Courtois catches. Chelsea are quickly back on the attack.
17' Despite the early Sunderland goal, Chelsea look in the mood today. The visitors have won another free-kick at the other end, though, with fouls aplenty in the opening exchances.
16' CHANCE! Another opening! David Luiz resists the temptation to shoot and picks out Diego Costa, who volleys just wide.
14' Another Chelsea free-kick as Rodwell again fouls Hazard. Sunderland will hope to defend this set-piece rather better than the last.
12' SAVE! This could be a long 90 minutes for Pickford and Sunderland. Chelsea again line up to shoot and Alonso thunders a strike towards goal, the keeper saving well.
12' CHANCE! Chelsea should be in front! Willian slips in Moses, who crosses towards Diego Costa in the centre. He fails to make contact and Hazard, with the goal gaping, cannot turn and prod the ball into the net.
10' Sunderland had 10 men in the box as Chelsea netted and that appears to be their gameplan to stifle the champions, with a wall of red and white shirts again forming in front of the hosts from the kick-off.

8' CROSSBAR! Oh so close to an equaliser! Marcos Alonso, formerly a Sunderland loan signing, curls a superb strike against the angle of crossbar and post from the set-piece.
7' Now Chelsea have a free-kick, though. Jack Rodwell upends Eden Hazard in a similar position to where Adnan Januzaj was fouled at the other end.

1' KICK-OFF: Sunderland get us under way!
John O'Shea leads out the visitors and, now, here come Terry and Chelsea! Kick-off is just a couple of minutes away.
The Sunderland side are in the tunnel, ready to head out onto the pitch and provide Chelsea with a guard of honour. The stands are bedecked with blue and white banners as the home fans get ready to party.
Diego Costa's hopes of taking the Golden Boot are surely over, however, with Harry Kane now six goals clear of the Chelsea front man.
"We have two targets: to try to win the 30 games in this league and to help Thibaut [Courtois] to win the Golden Glove," Conte said this week. Courtois has kept 16 clean sheets in the league this season - one more than Tottenham's Hugo Lloris at the top of the standings.
Victory today would make Chelsea the first Premier League side to record 30 wins in a 38-game campaign. Number 29 was a thriller - 4-3 against Watford - and the home crowd will hope for a similarly entertaining contest as the curtain falls on a successful season.
A Cesc Fabregas goal was decisive as Chelsea won the reverse clash 1-0 in December, while the Blues were winners in this fixture on the final day of the 2014-15 season, again topping the table. Sunderland have won on two of their last six visits to the Bridge, though, despite a decade largely made up of struggle in the top tier.
Moyes matches Chelsea's back three as Joleon Lescott is handed a first start for Sunderland. Jermain Defoe, hotly tipped for an off-season move, misses out through injury.
After making a number of changes for Monday's match against Watford, Conte recalls his star men, including top-scorer Diego Costa. John Terry, set to leave the club in the coming transfer window, leads Chelsea out on home turf one last time.

Chelsea will today lift the Premier League trophy for a fifth time, also matching Sunderland's overall tally of six top-flight titles after vastly contrasting campaigns for the two clubs. Results elsewhere cannot impact either side's fortunes; David Moyes' men will finish bottom of the table.
