The Netherlands international has an expiring contract with the Rossoneri, but the Serie A giants are keen to hold on to Van Bommel, and have offered him a new contract until June 2013, worth €2 million.
The 35-year-old has been heavily linked with a return to PSV in recent weeks, and reports from Holland suggest that he has already agreed terms with the Eindhoven side.
However, Van Bommel has yet to confirm whether he will return to the club where he previously played from 1999 until 2005.
The veteran joined Milan from Bayern Munich in January 2011 and has since developed into a key player at the San Siro outfit.
