The 27-year-old joined the Welsh outfit from fellow Premier League club West Brom for an undisclosed fee in January 2010.
He still had 10 months remaining on his current deal, but has terminated his contract by mutual consent as he does not feature in manager Michael Laudrup's first-team plans.
Moore made 20 appearances and scored five goals in all competitions for the Liberty Stadium outfit last season.
But his opportunities were set to be limited further in 2013-14 following the arrival of Ivory Coast international striker Wilfried Bony from Vitesse for a club-record fee in the close-season.
The former England Under-21 international began his career at Aston Villa before making the move to The Hawthorns in May 2008.
