The England international winger is set to end his largely unsuccessful spell at Anfield, with a transfer in the region of £5 million to be confirmed by Tuesday
Stewart Downing is undergoing a medical at West Ham as he closes in on a move from Liverpool, Goal understands.
The winger will complete a move to the London club in the next 24 hours if he passes the medical tests.
West Ham have agreed a fee of around £5 million with the Reds for the England international, who turned 29 in July.

Downing is set to become the third player to be offloaded by the Merseysiders to Upton Park this year, following in the footsteps of Andy Carroll and Joe Cole.
He has been deemed surplus to requirements at Anfield by manager Brendan Rodgers as Liverpool continue to pay the price for their signings in 2011.
Liverpool have already bought forwards Iago Aspas and Luis Alberto from Spain this summer and are still understood to be in the market for attacking talent as huge doubt remains over the future of prize asset Luis Suarez.
Downing was captured from Aston Villa in the summer of 2011 in a deal worth up to £20m but he has failed to establish himself on Merseyside.
